If your child needs medication to be administered in school, a form must be completed beforehand. Please complete the appropriate online form which will email the details directly to us.
Alternatively you can download the appropriate Word document to fill in electronically and email it back to us at parkside.primary@eastriding.gov.uk, or you can print it out to fill in and bring to school.
